This guide has been prepared to assist with the completion of the enrolment form. Please PRINT clearly when entering information on this form.
When completing the form you must ensure you enrol in all units you wish to study for 2008. All sections on the form must be completed to be valid. Forms that are not complete will be returned.
Personal Details
Check that the details shown are correct. Please notify the Student Centre if you notice any errors.
Enrolment Details
Course
Your course code, course title and Majors (if applicable) will be pre-printed.
Graduation Details
You need to advise the University when you expect to graduate. Write the year and semester you intend to graduate by and if you expect to attend graduation ceremonies. For example if you are completing a 3 year degree part time you would expect to graduate in 6 years. You can always update this information as you progress with your course.
Pre-printed Units
Some forms will be preprinted with units you must choose from.

Select the units you wish to study by entering the semester, attendance and study centre details. Where there is no choice these sections will be pre-printed.

Draw a line through any pre-printed units you do not wish to study.
Unit Codes
Add units you wish to study as part of your course by selecting them from the Course and Unit handbook.
Unit Titles
Note the title that corresponds to the unit code. If the unit code and title do not match your enrolment in that unit will not be valid and cannot be processed.
Semester
Indicate in which semester you wish to study the unit:
Su = Summer School: December to February
1 = Semester 1: February to June
W = Winter School: June to July
2 = Semester 2: July to October
Sp = Spring School: November to December
Attendance
Enter the mode of attendance:
A - On campus
F - Postgraduate
D - Distance (no on campus study)*
* If you are studying as an external student and expect to receive formal study materials by post, it is essential that you enter your Attendance as 'D'.
Study Centre
The study centre is where each unit is held. Valid choices are listed in the Course and Unit handbook.
Sch Approval
You can obtain School Approvals at the Enrolment Advice Days. If the Course and Unit information indicates that approval is required you must have this prior to submitting your form to the Student Centre. If you have a summer school unit then you must obtain approval.
Unistart
Indicate if you wish to enrol in Unistart. If yes you will need to indicate whether you plan to undertake it face to face or online and the Campus you wish to study it on.
Conditions of Offer
This section will show:

your status under the Higher Education Support Act,

any conditions attached to your offer of enrolment, and
-
any credit that has been assessed (if applicable).
Declaration and Student Signature
You need to read and understand the declaration and MUST sign the form before submitting it to the University. You need to be aware that the relevant laws will be binding on you and that illegal activities are expressly forbidden.
Approval of your enrolment can be obtained at the Enrolment Advice Days. The Sub-Dean or Faculty Officer will sign your form to indicate your enrolment is approved.
Variation of Address Details
Please indicate any changes to your residential address, postal address and contact phone numbers and supply details of someone to contact in an emergency. Your residential address should be your primary address of residence and the postal address is the address that you need to have correspondence sent to during the semester. If they are the same you only need to show the residential address.
NOTE: You cannot have a P O Box as your residential address.

Indicate if you have a disability that may affect your studies and whether you need advice and|or support.
