Where applicable, fill in your Year 12 Student Number (including interstate applicants), AMC Student Number, International Baccalaureate Number and/or your University of Tasmania Student Number. Postgraduate applicants are not required to provide their Year 12 Student Number.

List the courses for which you are applying in order of preference (1 is the most preferred course). When choosing your preferences, check that you are able to satisfy prerequisites for the courses. Course prerequisites are listed in the Course and Unit handbook http://courses.utas.edu.au. Do not list a course for which you would not accept an offer.

Outlines should comprise: (1) a description of the unit from the relevant year Handbook; (2) weighting/credit point value; (3) assessment e.g. exam (70%), practical work (30%); and (4) teaching pattern e.g. Semester 1 with 3 hour lectures and 1 hour tutorial per week; and
- a copy of the other institution’s course structure for the same degree e.g. BA. The course structure should advise of the other institution’s credit point/weight system (e.g. to complete a BA degree at Uni of Tas, students will enrol in units totalling 100% weight in each year of the three year course (total 300%)).
- Applicants to Australian Maritime College (AMC) seafaring courses who are requesting Credit (i.e. exemption) should provide certified copies of any STCW95 Short Course Certificates and Certificates of Competency (including endorsements).

Section D - Educational Level of your parents or guardians

These questions are about the highest level of education completed by your parents or guardians. The Australian Government Department of Employment, Education and Workplace Relations require this information to be collected for statistical purposes. It will not be used by the University in the assessment of your application for admission.

Section E - Documents/Additional Information

What documents do I need to supply with my application? |
Year 12 applicants |
We are usually able to access your Year 12 results electronically and you do not need to provide them. If for any reason we are unable to obtain your results, you will be contacted by the Admissions Office.

Non-Year 12 applicants |
We require certified copies of transcripts and/or completion certificates for studies undertaken at any TAFE or AQF Recognised Training Organisation, College of Advanced Education (CAE) or overseas institution (including year 12/13). The University does not generally require academic records from Australian universities. However, if we are unable to obtain these electronically we will send you a Request for Supporting Documents.

Applicants to Australian Maritime College seafaring courses |
Applicants who intend to use their qualification to undertake Australian Maritime Safety Authority Orals to gain a Certificate of Competency should visit http://www.amsa.gov.au/ to check seatime requirements detailed in Marine Orders Part 3. Please provide certified copies of any STCW95 Short Course Certificates and Certificates of Competency (including endorsements) for which you are requesting Credit (exemption). |
It is essential that you supply the correct documentation in support of your application. Applicants who do not meet the University’s General Entry Requirements (see the Admission Guide) must include a supporting statement addressing the selection criteria.
The onus for the provision of documentation rests with the applicant. You may be asked to produce original documents if you receive an offer of a place. Where this is the case your offer will make it clear that your place is provisional until the required documents have been sighted.
As assessment of applications begins immediately after the closing date, complete documentation must be submitted with your application form. Applications that do not include the required documents will be placed on hold and assessed as late documents are received. In such cases it is your responsibility to send us the transcript of your 2011 results as soon as they become available. The transcript should be clearly marked with your full name and date of birth.
All applicants must comply with the following:
- If you have changed your name since you undertook previous study we will need certified evidence of the name change, e.g. a marriage certificate or statutory declaration.
- If your documents are not in English you must provide an officially certified translation together with certified copies of the original documents. Translations based on photocopies are not acceptable.
- Applicants born outside Australia who have become Australian citizens or New Zealand citizens, or who hold either Australian Permanent Residency or Australian Humanitarian Visas, must supply a certified copy of their citizenship, certificate, passport or visa documents.
- All documents must be either originals or copies of originals certified by a Commissioner of Declarations, a Justice of the Peace, a university or college Registrar or other recognised authority such as a Police Officer, Pharmacist or Post Office Official, and must be submitted with the 2011 Application for Admission (except for 2010 non-Australian tertiary results which must be forwarded as soon as available). Note: Your original documents will not be returned unless you enclose a stamped self addressed envelope.
- To ensure your documents are correctly matched to your application, clearly mark your name on each page. Staple documents once only in the top left corner. Academic transcripts and visa/citizenship documentation must be certified (colour copies are not necessary).
You also need to supply documents if you:
- Are seeking entry as a non-year 12 applicant and do not meet the General Entry Requirements detailed in the Admission Guide, in which case you must provide a Supporting Statement addressing the selection criteria. If you have been excluded from any course, at any time, it is important that you also include an explanation that explains the circumstances surrounding your academic history, the activities you have undertaken since your exclusion, and the plans you have put in place to prevent a recurrence.
- Have undertaken any TAFE/AQF study whether completed, incomplete or current.
- Postgraduate applicants are required to provide a brief statement outlining their interest in undertaking the course of their study being applied for. A statement is not required for applicants to Honours programs, the Master of Architecture, Master of Education or Graduate Diploma in Legal Studies who have completed the relevant undergraduate studies at UTAS.
- Have changed your name since undertaking previous study.
- Have completed the International Baccalaureate but have not authorised the IB Organisation to forward results directly to us, or other overseas qualifications.
Please note:
A number of courses require specific documentation and/or a portfolio of work or attendance at an interview or audition. Please refer to http://www.studentcentre.utas.edu.au/admissions/forms_files/index.html for details.
Applicants for courses in Visual and Performing Arts will be advised by the relevant school of portfolio/audition requirements. Visual Art applicants that are asked to attend an interview should bring original work or digital images if the work is too large or delicate to transport.
29 Supporting Statement
Undergraduate applicants
You may be required to provide a supporting statement with your application if you are not a Year 12 applicant. This may be completed as part of the online application or submitted separately. Applicants who are required to submit a supporting statement must address the sections listed below. If you are including a resume that provides all the information asked for in the sections on Academic and Employment Background, you do not need to respond to those sections again.
Postgraduate applicants
Postgraduate applicants will be required to complete a brief statement outlining their interest in undertaking the course of study being applied for. A statement is not required for Honours, Master of Architecture, Master of Education or Graduate Diploma in Legal Practice if the relevant undergraduate studies were completed at UTAS.
Applicants are expected to write their own supporting statement.

Failure to provide a supporting statement will delay the assessment of your application.
Academic Background
- Include details of your educational background not provided in your application for admission – e.g. professional development courses, first aid qualifications or other vocational education. Include details of all courses whether they are in similar or different areas to the course you have applied for, and whether or not they have been completed.
- If you have unsuccessfully attempted university study, please provide a brief statement detailing the reasons why you wish to recommence study. If you have ever been excluded from a course or institution, it will be necessary for you to provide a re-admission statement which explains the circumstances surrounding your academic history, your activities since the exclusion and the plans you have put in place to prevent a recurrence.
Employment Background
- Provide concise details of your employment history including the duties, level of responsibility and the length of time in each position. Please mention any work experience that is relevant to the course you are applying for.
Knowledge & Skills
- Describe the practical skills you have gained through experience or study in a particular field. This does not have to be related to the course(s) you have applied for, and may include community work, voluntary service or hobbies.
- Provide information about any specialist knowledge you may have developed through experience or study in a particular field.
- Describe your communication and interpersonal skills and how they may assist you in your studies.
Demonstrated Interest
- Explain why you are interested in undertaking the course(s) and how this interest developed.
- Explain how you have prepared for university and what your career plans and goals are on completion of the course.
Equity or Special Consideration (if relevant to you)

Please provide concise details of any factors or circumstances which you feel may have limited your opportunities for education or access to a university, e.g. restricted educational opportunity, disability, health conditions, indigenous or non English speaking background.
30 Academic transcripts
If you are sending copies of academic transcripts from TAFE/AQF or an overseas university they must be certified as a true copy of the original by a Commissioner for Declarations, a Justice of the Peace, a university or college Registrar or other recognised examining authority.
Academic transcripts must show all subjects attempted, grades achieved (including failures) and awards conferred. Please do not send semester or annual result slips.
The University does NOT require the following documentation:
- Year 12 TCE/HSC results from Tasmanian applicants who undertook their studies after 1983;
- Year 12 results from interstate applicants who undertook their studies after 1991;
- Academic records for study at an Australian university. If you studied at another Australian university we will, in most cases, obtain your record electronically (as authorised by you in the Declaration on the 2011 Application for Admission). If we cannot obtain your record electronically we will contact you requesting a certified copy;
- Semester or annual result slips;
- Personal or character references, photographs, psychological or intelligence test assessments, course outlines or curricula (unless applying for credit), examples of work (except for Visual and Performing Arts); or
- Evidence of your date of birth with your application.

You should read the declaration and MUST sign before sending it to the University. Applications with unsigned declarations cannot be processed.

If you need to complete a supporting statement, or provide other documentation, please ensure that you send it to the Admissions office as soon as possible. To ensure your documents are correctly matched to your application, clearly mark your name on each page. Staple documents once only in the top left corner. Academic transcripts and visa/citizenship documentation must be certified (colour copies are not necessary).
If you would like confirmation of receipt of your application by post, you should enclose a stamped self addressed envelope with your application. This will be mailed to you once your application is received.
Do not fax your application. The University requires all applications to be submitted either online or on the original forms as your signature is required on the declaration. The only occasion that you may use a fax is if you think your application may be late. In this case you should send us a fax to indicate that your application has been posted by Priority Post before the closing date. In this way your application will be treated as ‘on-time’.
